The Ask, Implement, and Follow-Up approach is a way of providing effective and client-centered services to people experiencing homelessness. It means that we:
- Ask the client about their needs, preferences, goals, and strengths, and listen to their feedback and input.
- Implement a personalized plan of action that addresses the client’s housing, health, and social needs, and connects them to appropriate resources and supports.
- Follow-Up with the client regularly to monitor their progress, celebrate their achievements, and adjust the plan as needed.
